NOTICE
This form MUST be filled out for any event/function planned at St. Patrick Parish. No event gets placed on the parish calendar until it is approved by Fr. John. Requests must be submitted to the office NO LATER than 2 MONTHS prior to the proposed date.

Should your event be cancelled after event is placed on calendar, please contact the Parish Office as soon as possible. This way, the event can be removed from the calendar.
